Behavior observation sheet

Record what you can see before interpreting why it happens.

An original PetStory note-taking aid, not a validated assessment, diagnosis or treatment plan. Observe ordinary events only. Do not provoke barking, force confinement or leave a distressed pet alone to fill out this sheet. Injury or concerning changes warrant veterinary advice.

Fictional teaching example

A delivery cart passes the window. The dog barks while looking outside. The owner closes the curtain; the dog turns away. This records a sequence, not a diagnosis or proof that a treatment works. It is not a customer case.
